The following types of activities must complete and submit this form a minimum of 5 days prior to the event:

  • Plays, musicals, or theater productions
  • Concerts or band performances
  • Lectures or speeches
  • Dance performances
  • Comedy performances
  • Food festivals
  • Arts and entertainment festivals
  • Graduations
  • Any other event that exceeds the social gathering limits

Notification is required only for events exceeding New York State's non-residential social gathering limits of 100 people indoors or 500 people outdoors.
PLEASE NOTE: Indoor limit will increase to 250 people on May 19.

 

The current limits on numbers of attendees:

Indoors Small/medium venues <1,500 capacity Up to 100 people with no testing or 101-150 people with testing or proof of vaccination; up to 33% of capacity.
Starting May 19: Up to 250 without testing, >250 with testing or proof of vaccination; capacity limited by required social distance of 6 feet.
Large venues >1,500 capacity Up to 10% of capacity; testing or proof of vaccination required for all.
Starting May 19: Up to 30% of capacity; testing or proof of vaccination required for all.
Outdoors Small/medium venues <2,500 capacity Up to 500 without testing, >500 with testing or proof of vaccination; capacity limited by required social distance of 6 feet.
Large venues >2,500 capacity Up to 20% capacity; testing or proof of vaccination required for all.
Starting May 19: Up to 33% capacity, 6 feet of social distancing required; testing or proof of vaccination required for all. May designate a separate section for vaccinated people that can be up to 100% capacity with no social distancing requirements.
